As a pro se or self-represented party, you will need the full Dissolution of Marriage packet or other family law forms packet completed in order to file your case if a settlement is reached. If you both agree to the terms of your settlement during mediation, once that portion of the session is concluded and my obligation as your neutral mediator is finished, I can assist you in completing the required documents and family law forms you'll need to file your case so at the end you just need to sign/notarize each document and file at your local Clerk of Court. This is included in the cost of mediation. For parties represented by council, you may choose to have them or us complete the petition and supporting documentation for filing.

- Petition to Determine Paternity and for Related Relief
- Notice of Social Security Number (both parties)
- Designation of Current Email and Mailing Address (both parties)
- Uniform Child Custody Jurisdiction and Enforcement Act Affidavit
- Financial Affidavit (both parties complete this yourself)
- Child Support Guidelines Worksheet
- Motion to Deviate from Child Support Guidelines (if waiving support or modifying more than 5% from the guidelines)
- Parenting Plan
- Mediated Settlement Agreement
- Petition for Dissolution of Marriage
- Settlement Agreement for Parties Who Were Never Married with Minor Children
- Proposed Final Judgment of Paternity
